The children's Day Treatment program accepts referrals from all sources including self-referrals, primary care physicians, schools, the Department of Youth and Family Services, and other social service agencies.
Once a referral is made or a parent/guardian expresses interest in the program, an assessment will be scheduled to determine if the child is appropriate for the program. Families Forward will gather information from the child, caregivers, schools, medical personnel and social service agencies to complete a comprehensive assessment.
If a child is deemed eligible for the program they will be asked to visit the program and tour the facilities. Staff members will discuss the program policies with the family and provide the child's parent(s)/guardian(s) with information about enrollment in the program.
